JACKSONVILLE, FL. Island Wing Grill and Bar on Southside Boulevard drew 12 high-severity violations during a single inspection the week of September 17, including failures in parasite destruction procedures, inadequate shell stock records, and documented evidence that employees were not reporting illness symptoms before handling food.
The restaurant's inspection stood as the most alarming single finding in a week that saw state inspectors flag 15 Jacksonville facilities for high-severity violations, touching nearly every major food safety risk category on the books.
The Worst of the Week
Island Wing Grill's inspection record included a person in charge not present or not performing duties, no written employee health policy, employees not reporting illness symptoms, inadequate handwashing, improper handwashing technique, food in poor condition, inadequate shell stock identification, and parasite destruction procedures not followed. That is eight distinct categories of high-severity failure in a single visit, plus four more.
Krystal Restaurant on Normandy Boulevard was not far behind, with 10 high-severity violations. Inspectors cited the fast food location for food sourced from an unapproved or unknown supplier, food not cooked to required minimum temperatures, food contact surfaces not properly cleaned or sanitized, and no consumer advisory posted for raw or undercooked foods.
Dick's Wings and Grill on Old St. Augustine Road drew 8 high-severity violations, among them improperly stored toxic chemicals, no allergen awareness demonstrated, and no written employee health policy. Inspectors also cited the location for toxic substances improperly identified, stored, or used, a separate citation from the chemical storage violation.
JJ Wing and Burger on Duval Station Road matched Dick's Wings at 8 high-severity violations. The findings included food from an unapproved or unknown source, food not cooked to required minimum temperature, improperly stored toxic chemicals, and food contact surfaces not properly cleaned or sanitized.
Across the City
Banh Mi and Pho Express on Saint John Bluff Road South collected 7 high-severity violations, including parasite destruction procedures not followed, food in poor condition, and an intermediate citation for improper sewage or wastewater disposal, one of the more unusual findings of the week.
Secret Garden Cafe on Beach Boulevard also drew 7 high-severity violations. Inspectors found no person in charge present, no written employee health policy, no allergen awareness, toxic chemicals improperly stored, and an intermediate citation for improper sewage or wastewater disposal.
Twin Peaks Restaurant on Atlantic Boulevard was cited for 7 high-severity violations including food contaminated by chemical, physical, or biological hazards, inadequate handwashing by employees, and food not cooked to required minimum temperature. Inspectors also found shell stock identification records inadequate.
Flame Sushi Japanese Grill and Ramen on Beach Boulevard drew 7 high-severity violations, including food sourced from an unapproved or unknown supplier, food in poor condition, no consumer advisory for raw or undercooked items, and improperly stored toxic chemicals. The sushi restaurant's failure on the consumer advisory is particularly pointed given the volume of raw fish it serves.
McDonald's on North Main Street was cited for 7 high-severity violations, including shell stock identification failures, parasite destruction procedures not followed, toxic chemicals improperly stored, no consumer advisory for raw or undercooked foods, and no allergen awareness demonstrated.
Jville Seafood and More on Blanding Boulevard drew 6 high-severity and 7 intermediate violations, the highest intermediate count of the week. Inspectors cited the seafood restaurant for food in poor condition, improperly stored chemicals, no allergen awareness, improper sewage disposal, and multi-use utensils not properly cleaned.
St. James Cafe on West Duval Street was cited for 6 high-severity violations including toxic chemicals improperly stored, no allergen awareness, no consumer advisory for raw or undercooked foods, and single-use items improperly reused.
Super Wings and Philly on Wilson Boulevard drew 6 high-severity violations, among them parasite destruction procedures not followed, required procedures for specialized processes not followed, inadequate cold holding equipment, and shell stock identification records missing.
LongHorn Steakhouse on Airport Road was cited for 6 high-severity violations including food not cooked to required minimum temperature, no consumer advisory for raw or undercooked foods, toxic chemicals improperly stored, and an intermediate citation for single-use items improperly reused.
Latin Creations on St. Johns Bluff Road South drew 2 high-severity violations, both in the illness-reporting category: no person in charge performing duties and employees not reporting symptoms of illness.
Empanada's Factory on Philips Highway drew a single high-severity citation for food contact surfaces not properly cleaned or sanitized.
What These Violations Mean
The most pervasive finding this week was employees not reporting illness symptoms, cited at Island Wing Grill, Krystal, Dick's Wings, JJ Wing and Burger, Secret Garden Cafe, Twin Peaks, Flame Sushi, Jville Seafood and More, St. James Cafe, LongHorn Steakhouse, and Latin Creations. When a food worker who is sick with Norovirus continues handling food without reporting symptoms, they become a direct transmission route to every customer served that shift. Norovirus is responsible for roughly 20 million illnesses in the United States annually, and it spreads with exceptional efficiency from contaminated hands to food to surfaces.
Closely related is the absence of written employee health policies, documented at Island Wing Grill, Krystal, Dick's Wings, Banh Mi and Pho Express, Secret Garden Cafe, and Twin Peaks. A written policy is the mechanism that tells workers which symptoms require them to stay home. Without one, the illness-reporting failure is structural, not just individual.
The chemical storage violations this week carry a different kind of risk. Dick's Wings, JJ Wing and Burger, Banh Mi and Pho Express, Secret Garden Cafe, Flame Sushi, McDonald's on North Main, Jville Seafood, St. James Cafe, Super Wings and Philly, and LongHorn Steakhouse were all cited for toxic chemicals improperly stored or labeled. Several of those, including Dick's Wings, JJ Wing and Burger, St. James Cafe, McDonald's, and LongHorn Steakhouse, drew a separate citation for toxic substances improperly identified, stored, or used. Cleaning chemicals stored near food or mislabeled can cause acute poisoning that is indistinguishable from foodborne illness until the source is traced.
Parasite destruction failures at Island Wing Grill, McDonald's on North Main, Super Wings and Philly, and Banh Mi and Pho Express point to a specific protocol gap. Fish served raw or undercooked must be frozen to precise temperatures for a required duration to kill parasites including Anisakis and tapeworm. When that step is skipped or undocumented, the risk passes directly to the customer.
The Longer Record
LongHorn Steakhouse on Airport Road carries 52 prior inspections on record, the longest history of any facility in this week's data, and still drew 6 high-severity violations. Twin Peaks on Atlantic Boulevard and Latin Creations on St. Johns Bluff Road South each have 39 prior inspections. Super Wings and Philly on Wilson Boulevard also has 39. Facilities with that volume of inspection history have had extensive opportunity to correct recurring deficiencies.
Empanada's Factory on Philips Highway has 37 prior inspections on record and drew only a single high-severity citation this week, the best outcome among the long-tenured facilities. Island Wing Grill and Banh Mi and Pho Express both carry 35 prior inspections, yet Island Wing Grill produced the worst single-visit result of the week at 12 high-severity violations.
JJ Wing and Burger on Duval Station Road stands out from the other direction. It has only 4 prior inspections on record, making it among the newest locations in this week's group, and it already drew 8 high-severity violations including food from an unapproved source and improperly stored toxic chemicals. Jville Seafood and More on Blanding Boulevard has 8 prior inspections and drew 6 high-severity and 7 intermediate violations, the highest combined count among newer entrants.
Krystal on Normandy Boulevard has 31 prior inspections and this week produced 10 high-severity violations, including the food-from-unapproved-sources citation. That violation means some ingredient at the Normandy location arrived outside the USDA and FDA inspection chain this week, and if a customer becomes ill, tracing the source becomes substantially harder.
