JACKSONVILLE, FL. Iso Iso Ramen on San Jose Boulevard drew 11 high-severity violations during the week of June 17, the highest single-facility count among 15 Jacksonville restaurants flagged for serious food safety failures, with inspectors documenting food from unapproved or unknown sources, food not cooked to required minimum temperatures, and no person in charge present or performing duties.
Two other Jacksonville restaurants did not make it through the week open at all. Tunis Wing and Seafood at 1935 N. Main St. was emergency-closed June 19 after inspectors documented roach activity on site. Food Doctor at 2356 Beaver St. was shut down June 17 after inspectors found no potable water.
The Violations
Iso Iso Ramen's 11 high-severity citations also included inadequate shell stock identification records, food contact surfaces not properly cleaned or sanitized, an employee not reporting symptoms of illness, and improper hand and arm washing technique. The ramen shop had 26 prior inspections on record before this week's visit.
Mariscos El Pacifico on University Boulevard West drew nine high-severity violations, including food from unapproved sources, parasite destruction procedures not followed, inadequate handwashing by food employees, and toxic chemicals improperly stored or labeled. The seafood restaurant also had no employee health policy and no person in charge present during the inspection.
La Nopalera Mexican Restaurant on Philips Highway was cited for eight high-severity violations, including food not cooked to required minimum temperatures, required procedures for specialized processes not followed, and no consumer advisory for raw or undercooked foods. Inspectors also documented toxic substances improperly identified, stored, or used.
China Wok on Deerwood Lake Parkway pulled seven high-severity citations with no intermediate violations, a combination that points to systemic failures rather than maintenance lapses. Inspectors flagged parasite destruction procedures not followed, no allergen awareness demonstrated, and toxic chemicals improperly stored or labeled, alongside food contact surfaces not properly cleaned.
Miya Asian Fusion on Old St. Augustine Road also drew seven high-severity violations, including food contaminated by chemical, physical, or biological hazards, parasite destruction procedures not followed, and time as a public health control not properly used. Toxic chemicals were also improperly stored or labeled.
Six Facilities, Six Serious Findings
Canopy Road Cafe on Old St. Augustine Road was cited for six high-severity and six intermediate violations, the highest combined intermediate count of any facility this week. High-severity findings included inadequate shell stock identification records, parasite destruction procedures not followed, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored. Inspectors also documented improper sewage or wastewater disposal and multi-use utensils not properly cleaned.
Uncle Willies Southern Bistro on Dunn Avenue drew six high-severity violations, including food from unapproved or unknown sources, an employee not reporting symptoms of illness, and toxic chemicals improperly stored or labeled. Inspectors also noted improper sewage or wastewater disposal and single-use items being improperly reused.
Mellow Mushroom Pizza Bakers on Max Leggett Parkway collected six high-severity citations, including no allergen awareness demonstrated, dual toxic substance violations covering both improper storage and improper identification, and no consumer advisory for raw or undercooked foods. Inadequate cooling equipment was flagged as an intermediate violation.
Medusa Lounge on Bay Meadows Road drew five high-severity violations including inadequate handwashing facilities, meaning the physical infrastructure for hand hygiene was not in place. Inspectors also documented improper sewage or wastewater disposal and inadequate cooling equipment as intermediate violations.
Bagels R Us No. 2 on San Jose Boulevard was cited for five high-severity violations including inadequate shell stock identification records and no consumer advisory for raw or undercooked foods, an unusual pairing for a bagel shop that suggests the menu extends well beyond its name. Improper sanitizing solution or procedures and single-use items improperly reused added to the intermediate count.
Tunis on Soutel Drive, separate from the emergency-closed Tunis Wing and Seafood on N. Main Street, drew five high-severity violations including food from unapproved or unknown sources, no employee health policy, and toxic chemicals improperly stored or labeled. Inadequate ventilation and lighting was among the intermediate findings.
Min Garden on Baymeadows Road was cited for five high-severity violations including time as a public health control not properly used, food contact surfaces not properly cleaned, and no consumer advisory for raw or undercooked foods.
Los Loros Mexican Restaurant on Baymeadows Road drew four high-severity violations including inadequate shell stock identification records, no consumer advisory for raw or undercooked foods, and no person in charge present. Improper waste disposal, improper use of wiping cloths, and inadequate ventilation rounded out the intermediate findings.
Crazy Sushi on Deerwood Lake Parkway was cited for four high-severity violations including food from unapproved or unknown sources, food contact surfaces not properly cleaned, and toxic chemicals improperly stored. Inspectors also documented improper sewage or wastewater disposal and inadequate ventilation.
Krystal Restaurant on University Boulevard West drew four high-severity violations including food in poor condition, mislabeled, or adulterated, and toxic chemicals improperly stored or labeled. Improper sewage or wastewater disposal, inadequate ventilation, and equipment in poor repair were among the intermediate findings.
What These Violations Mean
Food from unapproved or unknown sources, cited at Iso Iso Ramen, Mariscos El Pacifico, La Nopalera, Uncle Willies, Tunis on Soutel, and Crazy Sushi, means inspectors could not verify where that food came from. If a customer gets sick, there is no supply chain to trace, no recall to follow, and no way to determine whether other customers ate from the same contaminated batch. USDA and FDA inspections exist precisely to catch Listeria, Salmonella, and E. coli before food reaches a kitchen.
Parasite destruction procedures not followed, documented at Mariscos El Pacifico, China Wok, Miya Asian Fusion, and Canopy Road Cafe, is a violation that carries real consequences for anyone eating raw or lightly cooked fish. Parasites including Anisakis and tapeworm survive in fish that has not been properly frozen before serving. Sushi and ceviche menus depend on that freezing step being completed correctly. When it is not documented, there is no way to know whether it happened.
The employee illness violations at Iso Iso Ramen, Mariscos El Pacifico, Uncle Willies, Mellow Mushroom, Medusa Lounge, Bagels R Us, Tunis on Soutel, Min Garden, and Los Loros represent a direct transmission risk. Norovirus, the most common cause of foodborne illness outbreaks in restaurant settings, spreads person-to-person through food handling. A written health policy, cited as absent at Mariscos El Pacifico, China Wok, Canopy Road Cafe, and Tunis on Soutel, is the minimum structural requirement for ensuring sick workers stay home.
Toxic chemicals improperly stored or labeled appeared at ten of the fifteen facilities flagged this week. That frequency is not coincidental. Cleaning and sanitizing chemicals stored near food preparation areas, or in unlabeled containers, create a contamination risk that is immediate and irreversible once a chemical reaches food.
The Longer Record
La Nopalera on Philips Highway carries 67 prior inspections on record, the longest history of any facility in this week's data, and still drew eight high-severity violations. Tunis on Soutel Drive shows 58 prior inspections and five high-severity violations this week, including the same food sourcing and handwashing failures that appear repeatedly in long-inspection-count facilities. Mariscos El Pacifico at 42 prior inspections drew the second-highest violation total of the week at nine high-severity citations.
Los Loros on Baymeadows Road has 37 inspections on record. Uncle Willies on Dunn Avenue has 33. Neither facility's history suggests the violations this week are anomalies.
Canopy Road Cafe on Old St. Augustine Road is the newest facility in this data, with only six prior inspections on record, yet it produced the highest combined violation total of the week at 12 across high-severity and intermediate categories. Improper sewage disposal and inadequate shell stock records in a facility this early in its inspection history is a different kind of concern than the same violations at a 67-inspection restaurant.
Krystal Restaurant on University Boulevard West carries 27 prior inspections. The finding of food in poor condition, mislabeled, or adulterated at a national fast-food chain with that many inspections on record remains unresolved in this week's data.
