JACKSONVILLE, FL. State inspectors cited Emperors Gentlemen's Club on University Boulevard West for 12 high-severity violations during the week of July 8, the highest single-facility count in Jacksonville that week, in a round of inspections that flagged 15 restaurants for serious food safety failures and shut down one Norwood Avenue seafood house entirely.

The emergency closure came first. On July 8, inspectors ordered CJ Crab House and Seafood at 5892 Norwood Ave shut down after documenting rodent, roach, and fly activity inside the facility. It was the only emergency closure in Duval County that week.

Babylon Restaurants Lounge and Bar on Baymeadows Road was cited for eight high-severity violations, including parasite destruction procedures not followed, food not cooked to required minimum temperature, and toxic chemicals improperly stored. Inspectors also noted no consumer advisory for raw or undercooked foods and no allergen awareness.

Checkers #3267 on Lem Turner Road drew eight high-severity violations including no person in charge present, no employee health policy, employees not reporting illness symptoms, and toxic substances improperly identified, stored, or used, in addition to improperly stored chemicals. The franchise location also lacked a consumer advisory for raw or undercooked foods.

Cantina Louie on Hospitality Lane was cited for eight high-severity violations including employees not reporting illness symptoms, food not cooked to required minimum temperature, and both toxic chemical violations simultaneously, improperly stored chemicals and toxic substances improperly identified or used.

Great Wall Chinese Restaurant on Fort Caroline Road drew eight high-severity violations, among them food from an unapproved or unknown source, food in poor condition or adulterated, and time as a public health control not properly used. Inspectors also found no allergen awareness demonstrated.

Coopers Hawk Winery and Restaurant on Big Island Drive received eight high-severity citations with no intermediate violations at all. Inspectors found no person in charge, employees not reporting illness symptoms, food from an unapproved or unknown source, parasite destruction procedures not followed, food not cooked to required minimum temperature, and time as a public health control not properly used.

Margaritas Mexican Grill on San Jose Boulevard drew seven high-severity violations and an intermediate citation for improper sewage or wastewater disposal. The high-severity list included no person in charge, employees not reporting illness symptoms, and no allergen awareness demonstrated.

Players Grille on San Jose Boulevard was also cited for improper sewage or wastewater disposal alongside seven high-severity violations, including improperly stored chemicals and toxic substances improperly identified or used.

Bono's Pit Bar B Q on Norwood Avenue, just blocks from the emergency-closed CJ Crab House, drew seven high-severity violations including no person in charge, employees not reporting illness symptoms, and both toxic substance violations. An intermediate citation noted multi-use utensils not properly cleaned.

Desert Rider Sandwich Shop on Hogan Street was cited for seven high-severity violations including food not cooked to required minimum temperature, time as a public health control not properly used, and inadequate shell stock identification records.

BurgerFi on Big Island Drive, adjacent to Coopers Hawk in the same commercial complex, drew six high-severity violations including parasite destruction procedures not followed, food not cooked to required minimum temperature, and time as a public health control not properly used. Inspectors also noted improper sewage disposal and inadequate cooling or cold holding equipment.

Fla Juice and Bowl on San Marco Boulevard drew six high-severity violations including no person in charge, employees not reporting illness symptoms, and time as a public health control not properly used. Inspectors also documented improper sewage disposal and multi-use utensils not properly cleaned.

Waffle House #1050 on J Turner Butler Boulevard drew six high-severity violations including employees not reporting illness symptoms, parasite destruction procedures not followed, and no allergen awareness demonstrated. Krystal Restaurant on University Boulevard West was the clearest outlier of the week, drawing zero high-severity violations and a single intermediate citation for improper sewage or wastewater disposal.

What These Violations Mean

The most common high-severity citation this week, appearing at more than a dozen facilities, was improper hand and arm washing technique. This is not a paperwork violation. Hands that are washed incorrectly, too briefly, without soap, or skipping the wrist and forearm, can still carry Norovirus, Salmonella, and E. coli directly onto food. Norovirus alone accounts for roughly 20 million illnesses in the United States each year, and food workers are its primary transmission route.

The employee illness violations at facilities including Emperors Gentlemen's Club, Checkers on Lem Turner, Margaritas Mexican Grill, Bono's Pit Bar B Q, and Coopers Hawk Winery represent a separate and acute risk. When no written health policy exists and employees are not required to report symptoms, a worker with active Norovirus or Hepatitis A has no formal barrier between their illness and the food they are handling. Outbreaks traced to a single sick employee have sickened dozens of customers in a single service period.

Food from unapproved or unknown sources, cited at Tasty Seafood Chicken and Subs, Great Wall Chinese Restaurant, and Coopers Hawk Winery, is a traceability problem as much as a safety problem. When a customer becomes ill, investigators trace the food back through the supply chain to find the contamination point. Food purchased outside licensed distributors has no chain of custody. If someone gets sick, there is no record to follow.

The parasite destruction citations at Babylon Restaurants, Coopers Hawk Winery, BurgerFi, and Waffle House #1050 concern fish served raw or undercooked. Parasites including Anisakis survive in fish that has not been frozen to required temperatures before service. Consuming them causes severe gastrointestinal illness. The consumer advisory violations at nearly every facility on this list compound the problem: customers who might choose to avoid raw preparations have no notice that raw or undercooked items are even on the menu.

The Longer Record

The facility with the longest inspection history on this week's list is Bono's Pit Bar B Q on Norwood Avenue, with 45 prior inspections on record. Seven high-severity violations at a location that has been through 45 inspections suggests these are not first-time oversights.

Margaritas Mexican Grill has 42 prior inspections on record, the second-highest count among this week's facilities. Seven high-severity violations and an improper sewage disposal citation at a location with that many prior visits puts the week's findings in a different light than a new establishment still learning its obligations.

Babylon Restaurants Lounge and Bar has 31 prior inspections and Coopers Hawk Winery and Restaurant has 33. Both drew eight high-severity violations this week. Coopers Hawk's citation for food from an unapproved source, at a full-service winery and restaurant with more than three dozen inspections behind it, is among the harder findings to explain as an administrative oversight.

BurgerFi and Fla Juice and Bowl each have 15 prior inspections on record, the lowest counts among this week's high-severity facilities. Both are relatively newer to the inspection cycle. BurgerFi drew six high-severity violations including inadequate cooling equipment, a structural deficiency that does not resolve itself between visits.

Margaritas Mexican Grill also had improper sewage or wastewater disposal on its citation list this week, a violation shared by Players Grille, BurgerFi, Fla Juice and Bowl, and Waffle House #1050. Five facilities with sewage disposal problems in a single week across Jacksonville is a number that has not been explained.
