JACKSONVILLE, FL. Five restaurants were emergency-closed in Jacksonville during the week of July 28, and one of them, Hooters on San Jose Boulevard, was shut down on the first day of that stretch after inspectors documented both rodent and fly activity on the premises.

Hooters was one of 15 Jacksonville facilities cited for high-severity violations during the same seven-day window. The restaurant's closure on July 28 came alongside a separate inspection that turned up one high-severity and two intermediate violations, including a missing consumer advisory for raw or undercooked foods. That the closure happened on the same date as the routine inspection suggests inspectors found the pest evidence during the same visit.

Cruz Mexican Grille at 13423 Beach Boulevard was ordered closed July 31 for roach activity. Mod Pizza at 12547 Bartram Park Boulevard was also shut down July 31, this time for fly activity. Soul Bowl Bistro at 6132 Merrill Road was closed a day earlier, on July 30, after inspectors determined there was no potable water on site. Captain D's at 2062 North Main Street was closed the same day as Hooters, July 28, for rodent activity.

Five emergency closures in seven days is a significant concentration for a single city in a single week.

The Violations

The facility with the most high-severity violations this week was Salsas Mexican Restaurant on West Beaver Street, which drew 13 high-severity and 6 intermediate citations. Inspectors documented that the person in charge was not present or not performing duties, that no employee health policy existed, that employees were not reporting illness symptoms, that handwashing was both inadequate in practice and undermined by inadequate facilities, and that food arrived from unapproved or unknown sources. Food in poor condition was also cited.

That combination at a single facility, absent management, no illness reporting policy, no functioning handwashing infrastructure, and uninspected food supply, represents nearly every vector through which a foodborne outbreak can begin.

Galaxy Burgers Bar and Grill on Beach Boulevard followed with 10 high-severity violations. Among the most serious: inadequate shell stock identification records, meaning shellfish on the menu could not be traced to an approved source if a customer became ill; parasite destruction procedures not followed for fish; and food contact surfaces not properly cleaned or sanitized. The restaurant also lacked a consumer advisory for raw or undercooked menu items.

Dunkin' on Atlantic Boulevard accumulated 9 high-severity violations, a count that would be notable at any restaurant but stands out at a chain operation where corporate food safety standards are supposed to be uniform. Inspectors found food from an unapproved or unknown source, inadequate handwashing by employees, improper handwashing technique, parasite destruction procedures not followed, food contact surfaces not properly cleaned, and time as a public health control not properly used.

Bowl of Pho on Old Baymeadows Road also reached 9 high-severity violations. The facility had no person in charge present, employees not reporting illness symptoms, improper handwashing technique, food from unapproved sources, inadequate shell stock records, parasite destruction failures, unsanitized food contact surfaces, and no consumer advisory for raw items.

Tala Bay Seafood and Grill on US 301 drew 8 high-severity violations and 7 intermediate ones, the highest intermediate count of any facility this week. Inspectors cited the absence of an employee health policy, employees not reporting illness symptoms, inadequate handwashing facilities, improper handwashing technique, food in poor condition, no consumer advisory, and two separate chemical storage violations: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used.

Mr Gordo's Tacos and Cantina on West University Avenue was cited for food not cooked to required minimum temperatures, a direct pathway for Salmonella survival in poultry, alongside no person in charge, no handwashing facilities, inadequate shell stock records, toxic substance violations, and required procedures for specialized processes not followed.

Okinawa Japanese Grillhouse and Sushi Bar on Roosevelt Boulevard accumulated 8 high-severity violations. Parasite destruction procedures were not followed, a finding that carries particular weight at a sushi operation where raw fish is a menu staple. Food contact surfaces were not properly cleaned, and two separate chemical violations were noted.

Deercreek Country Club on McLaurin Road North drew 7 high-severity violations including food not cooked to required minimum temperature and improper sewage or wastewater disposal, a combination that introduces both pathogen survival and fecal contamination risk in the same kitchen.

Potbelly Sandwich Works on Old Baymeadows Road was cited for 6 high-severity violations including parasite destruction failures, food not cooked to required minimum temperature, time as a public health control not properly used, and no allergen awareness demonstrated. Metro Diner on Baymeadows Drive matched that count with its own 6 high-severity violations, including improperly stored toxic chemicals, unsanitized food contact surfaces, and single-use items being reused.

What These Violations Mean

The most concentrated risk this week came from the cluster of employee illness reporting failures. Salsas Mexican Restaurant, Galaxy Burgers, Bowl of Pho, Tala Bay Seafood, Mr Gordo's, Heavenly Cafe, Potbelly, and Metro Diner were all cited for employees not reporting illness symptoms. When a sick food worker handles food without reporting, they become a direct transmission route for Norovirus, which causes roughly 20 million illnesses in the United States each year. The reporting requirement exists precisely because most people infected with Norovirus are contagious before they feel severely ill.

The food from unapproved sources violations at Salsas, Dunkin', and Bowl of Pho carry a different but equally serious risk. When food enters a kitchen from an uninspected supplier, there is no traceability chain. If a customer becomes ill, investigators cannot identify the source, cannot issue a recall, and cannot determine how many other restaurants received the same shipment.

Parasite destruction failures at Galaxy Burgers, Dunkin', Bowl of Pho, Heavenly Cafe, Okinawa, Potbelly, and Metro Diner matter most at operations serving raw or lightly cooked fish. The protocol requires freezing fish to specific temperatures for specific durations before serving it raw. Without that step, parasites including Anisakis and tapeworm remain viable in the flesh. Okinawa, as a sushi bar, faces the most direct exposure from this violation.

The dual chemical violations at Tala Bay, Okinawa, Deercreek Country Club, Seven Wonder Bakery, and Metro Diner point to a separate and underappreciated hazard. Cleaning chemicals stored near or above food preparation surfaces can contaminate food through spillage or mislabeling. Chemical poisoning from this route is acute, not gradual.

The Longer Record

Bowl of Pho has the longest inspection history of any facility in this week's roundup, with 48 prior inspections on record. Nine high-severity violations on that 49th visit suggests the facility has not used that history to establish durable food safety practices.

Okinawa Japanese Grillhouse and Sushi Bar has 41 prior inspections on record. Eight high-severity violations this week, including parasite destruction failures at a raw fish operation, place that history in an uncomfortable light.

Metro Diner and Seven Wonder Bakery and Catering on University Boulevard North each have 35 prior inspections. Seven Wonder drew citations this week for food in poor condition, unsanitized food contact surfaces, and improperly stored toxic chemicals. Thirty-five inspections is a long runway.

At the opposite end, Potbelly Sandwich Works has only 2 prior inspections on record, making this week's 6 high-severity violations, including allergen awareness failures and food not cooked to temperature, an early and serious signal for a location still establishing its operational baseline. Mr Gordo's Tacos and Cantina has 10 prior inspections and already carries citations for undercooked food and improper specialized process procedures.

Hooters, closed for rodents and flies on July 28, has 28 prior inspections on record. Whether those prior visits documented any pest activity remains an open question in the public record.
