A raw fish restaurant on Forum Boulevard served food from unapproved or unknown sources and had no parasite destruction procedures in place, inspectors found during the week of September 3, 2026, one of the most dangerous combinations possible at a poke restaurant where fish is eaten with minimal cooking.
Poke Hawaii at 3242 Forum Blvd drew three high-severity violations during the inspection period. Beyond the sourcing and parasite concerns, inspectors also cited the restaurant for improperly using time as a public health control, meaning food was allowed to sit in the temperature danger zone longer than regulations permit without proper documentation or tracking.
What Inspectors Found
The week's most cited facility was Saucy Meatball at 12401 Commerce Lakes Dr, which accumulated seven high-severity violations in a single inspection. The list included a person in charge not present or not performing duties, an employee not reporting symptoms of illness, inadequate shell stock identification records, food contact surfaces not properly cleaned or sanitized, improper use of time as a public health control, no consumer advisory for raw or undercooked foods, and toxic chemicals stored improperly or without proper labels.

What These Violations Mean
