That last citation is notable for an ice cream concept, suggesting shellfish ingredients or cross-contamination risk that inspectors flagged as inadequately documented.

Denny's at 5000 N Federal Hwy. drew five high-severity violations including two separate handwashing failures: inadequate handwashing facilities and improper hand and arm washing technique. Inspectors also cited the restaurant for food contact surfaces not properly cleaned or sanitized, inadequate shell stock records, and no consumer advisory for raw or undercooked foods.

Siam House at 1392 SE 17 St. had five high-severity violations including employee illness reporting failure, improper handwashing technique, food contact surfaces not properly cleaned, misuse of time as a public health control, and no consumer advisory for raw or undercooked foods. Siam House also drew a citation for multi-use utensils not properly cleaned.

Pura Vida Miami at 2364 N Federal Hwy. was cited for food from an unapproved or unknown source, food not cooked to required minimum temperature, employee illness reporting failure, toxic substances improperly identified or stored, and misuse of time as a public health control. The combination of an unapproved food source and undercooked food in the same inspection is among the more serious pairings in this week's data.

Offerdahl's Off-the-Grill at 2122 NW 62 St. drew four high-severity violations and three intermediate ones, the highest intermediate count of any facility this week. High-severity citations included no person in charge, employee illness reporting failure, improper handwashing technique, and misuse of time as a public health control.

Lobster Bar Sea Grille at 450 E Las Olas Blvd. was cited for four high-severity violations including toxic chemicals improperly stored or labeled, food contact surfaces not properly cleaned, no consumer advisory for raw or undercooked foods, and employee illness reporting failure. A seafood-focused restaurant on Las Olas with improperly stored chemicals and no consumer advisory for raw preparations is a combination inspectors flag as particularly consequential.

La Corte Cafe at 100 E Broward Blvd. had four high-severity violations including toxic chemicals improperly stored, no employee health policy, food contact surfaces not properly cleaned, and no consumer advisory. Inspectors also cited the cafe for improper sewage or wastewater disposal.

Chili's Grill and Bar at 6302 NW 9 Ave. drew four high-severity violations with no intermediate violations, including food from an unapproved or unknown source and inadequate handwashing by food employees. Noodles Panini at 821 E Las Olas Blvd. also reached four high-severity violations, with citations for no employee health policy, employee illness reporting failure, improper handwashing technique, and food not cooked to required minimum temperature.

Timpano Italian Chophouse at 450 E Las Olas Blvd., sharing a street address with Lobster Bar Sea Grille, drew three high-severity violations: no person in charge, employee illness reporting failure, and food from an unapproved or unknown source.

El Churrascaso Steakhouse at 1611 N Federal Hwy., California Pizza Kitchen at 2301 N Federal Hwy., Host Table and Tap at 1522 NE 4 Ave., and Jimmy John's at 1135 S Federal Hwy. each drew three high-severity violations. El Churrascaso was cited for food in poor condition and no consumer advisory for a steakhouse that presumably serves beef at varying temperatures. Jimmy John's, a sandwich chain not typically associated with shellfish, drew a citation for inadequate shell stock identification records alongside improper handwashing technique and food not cooked to required minimum temperature.

What These Violations Mean

The most frequently cited high-severity violation this week was employee illness reporting failure, documented at the Hyatt Place, Chill N Nitrogen, Siam House, Pura Vida Miami, Offerdahl's, Lobster Bar Sea Grille, Noodles Panini, and Timpano Italian Chophouse. When food workers do not report symptoms of illness, they become a direct transmission route for norovirus, the pathogen responsible for 20 million U.S. cases of foodborne illness annually. Offerdahl's Off-the-Grill has only 7 prior inspections and drew the week's highest combined violation count at four high-severity and three intermediate.

Chill N Nitrogen and Jimmy John's each have 16 prior inspections. The Hyatt Place hotel restaurant has 14. None of those three are new operations, and all three drew five or three high-severity violations respectively in a single week.

Noodles Panini, with 30 inspections and a citation this week for food not cooked to required minimum temperature, has the longest record of any facility on this list alongside a violation that represents one of the most direct pathogen survival risks in food service. That combination, across three decades of inspections, remains unresolved.
