FORT LAUDERDALE, FL. Phat Boy Sushi on North Federal Highway racked up seven high-severity violations in a single inspection last week, including inadequate handwashing facilities, food in poor condition, and improperly cleaned food contact surfaces, with no person in charge present and no consumer advisory posted for raw fish on the menu. It was the worst single-facility tally in a week when state inspectors cited 14 Fort Lauderdale restaurants for high-severity violations.
The Week's Worst Offenders
Lucky Cat Street Eatery on North Andrews Avenue followed with six high-severity violations, among them inadequate shell stock identification records for shellfish, no person in charge, and food contact surfaces that inspectors found improperly cleaned or sanitized. Two intermediate violations rounded out the report: improper use of wiping cloths and inadequate toilet facilities.
Bayview Diner on East Oakland Park Boulevard drew five high-severity citations, including no consumer advisory for raw or undercooked foods and inadequate shell stock identification, alongside four intermediate violations covering improperly cleaned multi-use utensils, single-use items being reused, and inadequate ventilation and lighting.
Bonefish Grill at Terminal Drive, located inside Fort Lauderdale-Hollywood International Airport, was cited for five high-severity violations including food not cooked to the required minimum temperature, no employee health policy, and inadequate shell stock identification records. Improperly cleaned multi-use utensils and inadequate ventilation rounded out the report.
Unapproved Sources and Undercooking
Three facilities drew the specific citation for food from unapproved or unknown sources, one of the most serious categories inspectors can document.
Cucina Moderna on North Federal Highway was cited for sourcing food from unapproved or unknown origins, along with improperly cleaned food contact surfaces, improper handwashing technique, and no consumer advisory for raw items. Inspectors also noted improper sewage or wastewater disposal, a citation that signals potential fecal contamination risk throughout the facility.
Marriott Fort Lauderdale North Bluefire Bar and Grille on North Andrews Avenue received the same unapproved food source citation, paired with food not cooked to the required minimum temperature, improperly cleaned food contact surfaces, and improper handwashing technique. Single-use items were found being reused.
Myapapaya on North Federal Highway was also cited for food from unapproved or unknown sources, combined with improper handwashing technique, employees not reporting illness symptoms, and inadequate shell stock identification records.
Pomperdale Deli on East Commercial Boulevard logged four high-severity violations: no employee health policy, employees not reporting illness symptoms, improper handwashing technique, and improperly cleaned food contact surfaces.
Pita Plus on Stirling Road drew three high-severity citations, including inadequate shell stock identification and a citation for required procedures for specialized processes not being followed, a violation that applies to techniques like smoking, curing, fermenting, and reduced-oxygen packaging. Improper sewage or wastewater disposal was also noted.
The week's two airport Einstein Brothers locations were both cited. Einstein Brothers Bagel at 200 Terminal Drive received three high-severity violations: no person in charge, employees not reporting illness symptoms, and no consumer advisory for raw or undercooked foods. Einstein Bros Bagel #1597 on North Federal Highway was separately cited for employees not reporting illness symptoms and inadequate shell stock identification records.
Bokampers at Terminal Drive was cited for no person in charge and improperly cleaned food contact surfaces. Pei Wei at 200 Terminal Drive drew the same food contact surface citation alongside employees not reporting illness symptoms. Las Vegas Restaurant #5 on East Oakland Park Boulevard was cited for no person in charge and employees not reporting illness symptoms.
What These Violations Mean
The illness-reporting violations documented at ten of the fourteen facilities this week, including Phat Boy Sushi, Lucky Cat Street Eatery, Bonefish Grill, Pomperdale Deli, Myapapaya, Pita Plus, Las Vegas Restaurant #5, Einstein Brothers Bagel, Einstein Bros Bagel #1597, and Pei Wei, represent the most direct transmission risk in food service. When employees do not report symptoms of illness, a single worker with norovirus can infect dozens of customers before anyone in management knows there is a problem. Norovirus is responsible for nearly half of all foodborne illness outbreaks in the United States.
The unapproved food source violations at Cucina Moderna, Marriott Bluefire Bar and Grille, and Myapapaya carry a different but equally serious risk. Food that bypasses USDA or FDA inspection has no documented chain of custody. If a customer becomes ill after eating at one of these restaurants, investigators have no records to trace the product back to its origin.
The shellfish traceability citations at Lucky Cat Street Eatery, Bayview Diner, Bonefish Grill, Myapapaya, Pita Plus, and Einstein Bros Bagel #1597 point to the same traceability gap, but for a specific high-risk product. Oysters, clams, and mussels are frequently consumed raw or lightly cooked, and without proper shell stock tags, there is no way to identify the harvest location or date if a customer reports illness.
The undercooked food violations at Bonefish Grill and Marriott Bluefire Bar and Grille are among the most direct pathogen risks in the dataset. Salmonella in poultry requires an internal temperature of 165 degrees Fahrenheit to be destroyed. Food served below that threshold is not a regulatory technicality; it is a delivery mechanism for bacterial illness.
The Longer Record
Several of this week's cited facilities are not newcomers to the inspection process, and the volume of their prior inspections makes the current findings harder to dismiss as isolated incidents.
Bayview Diner carries 32 prior inspections on record, the highest count in this week's group. Five high-severity violations and four intermediate citations at a facility that has been through the inspection process three dozen times suggests the problems documented this week are not the result of unfamiliarity with state standards.
Pomperdale Deli has 29 prior inspections on record. Las Vegas Restaurant #5 has 30. Lucky Cat Street Eatery has 28. All three were cited this week for illness-reporting failures, which are among the most fundamental employee health requirements in the state code.
Phat Boy Sushi, the week's top violator with seven high-severity citations, has 22 prior inspections on record. The absence of adequate handwashing facilities at a restaurant that has been inspected more than twenty times is a detail the record does not explain.
Cucina Moderna presents a different problem. With only three prior inspections on record, it is one of the newest facilities in this week's group, and it already carries four high-severity violations including food from unapproved sources and improper sewage disposal. New facilities with serious violations early in their inspection history are worth watching. Cucina Moderna's next inspection will show whether this week's findings prompted corrections.
