ORLANDO, FL. A downtown Orlando cafe was caught serving food from unapproved or unknown sources during a September 21 inspection, a violation that means the ingredients on customers' plates could not be traced back through any USDA or FDA inspection chain, and the restaurant was allowed to remain open.

City Beautiful Cafe at 400 S Orange Ave drew seven high-severity violations and two intermediate violations in the September 21 inspection, according to state records. No emergency closure order was issued.

9INTImproper use of wiping clothsCross-contamination

The unapproved food source violation is among the most serious a restaurant can receive. It means inspectors could not confirm that at least some of the food in the kitchen had passed through a licensed, inspected supplier. If a customer got sick, investigators would have no supply chain to trace.

Alongside that finding, inspectors cited the cafe for having no written employee health policy and for employees not reporting illness symptoms. Those two violations are listed separately in the record because they represent separate failures: one is the absence of a written system, the other is evidence that even without a written policy, workers were not self-reporting when sick.

Inspectors also found inadequate handwashing facilities and improper handwashing technique, a pairing that compounds the illness risk. Even when workers attempted to wash their hands, the technique was insufficient to remove pathogens.

Toxic chemicals were improperly stored or labeled, according to the inspection record. The cafe also lacked a consumer advisory notifying diners that raw or undercooked items were on the menu. Multi-use utensils were not properly cleaned, and wiping cloths were used improperly.

What These Violations Mean

The combination of no employee health policy and no illness reporting represents the most direct route from a sick kitchen worker to a sick dining room. Norovirus, which causes the majority of foodborne illness outbreaks in restaurant settings, spreads through exactly this mechanism. A worker who does not know they are required to report symptoms, at a facility with no written policy governing that requirement, has no institutional reason to stay home or to tell a manager.

The handwashing violations at City Beautiful Cafe reinforce that risk. Inadequate facilities means the infrastructure to wash hands properly does not exist in the kitchen. Improper technique means that even when workers tried, they were not removing pathogens effectively. Both violations were cited on the same inspection day.

Food from unapproved sources is a different category of danger. It is not about behavior inside the kitchen; it is about what enters the building before cooking begins. Without a licensed supplier, there is no inspection record, no lot number, and no recall mechanism if contamination is discovered later.

The improperly stored or labeled toxic chemicals violation carries an acute risk that is distinct from everything else on the list. Chemicals stored near food, or in unlabeled containers, can cause poisoning that has nothing to do with bacteria or viruses. It is immediate and direct.

The September 21 inspection was the ninth on record for City Beautiful Cafe. The facility has accumulated 61 total violations across those nine visits. It has never been emergency-closed.

Seven of those nine inspections produced high-severity violations. The only exception is the earliest inspection on record, from August 2023, which found zero high-severity citations. Every inspection since then has found at least four.

The September 21 count of seven high-severity violations is the highest single-day total in the cafe's inspection history. The follow-up inspection conducted the next day, September 22, still found four high-severity violations, meaning the cafe entered the day after its worst-ever inspection with serious problems unresolved.

There is no inspection in this record that shows a sustained correction. The violation counts rise and fall between inspections, but high-severity citations have appeared at every visit since August 2023.

City Beautiful Cafe has never received an emergency closure order across nine inspections and 61 total violations. As of the September 22 follow-up, it remained open.
