KEY WEST, FL. Inspectors visiting Carolines Cafe on Duval Street in July found toxic chemicals improperly stored and labeled near food, no functioning handwashing facilities, and food contact surfaces that had not been properly cleaned or sanitized. The cafe logged six high-severity violations and two intermediate ones on July 21, 2026. It was not closed.
What Inspectors Found
The two chemical violations are among the most immediately dangerous a food service inspector can cite. Records show the cafe had toxic chemicals that were improperly stored or labeled, as well as a separate citation for toxic substances improperly identified, stored, or used. Both violations were logged as high severity on the same inspection.
Inspectors also found no consumer advisory posted for raw or undercooked foods. That citation means customers had no way of knowing whether dishes on the menu carried elevated risk from undercooked proteins or raw ingredients.
The person in charge was either not present or not performing required supervisory duties. That finding sat alongside the discovery that handwashing facilities were inadequate, and that food contact surfaces, the cutting boards, prep counters, and utensils that touch every dish leaving the kitchen, had not been properly cleaned or sanitized.
The two intermediate violations added to the picture. Inspectors documented improper sewage or wastewater disposal, and found toilet facilities that were inadequate or improperly maintained.
What These Violations Mean
The two chemical violations are not paperwork problems. When toxic cleaning agents or pesticides are stored near food without proper labeling, a mislabeled bottle or a spill can contaminate food directly, with no visible sign and no smell to warn a customer. The risk is acute poisoning, not the gradual kind associated with spoiled food.
The handwashing failure compounds every other violation on the list. Without functional handwashing facilities, employees cannot perform basic hygiene between tasks. Paired with food contact surfaces that were not properly cleaned or sanitized, the inspection record describes a kitchen where bacterial transfer from surface to surface, and from employee hands to food, faced no structural barrier.
The sewage and toilet facility violations raise a separate concern. Improper wastewater disposal creates the possibility of fecal contamination reaching food preparation areas. Inadequate toilet facilities discourage employees from washing hands after restroom use, which loops directly back to the handwashing citation.
The missing consumer advisory matters most to the people least able to absorb the risk: elderly diners, pregnant women, young children, and anyone with a compromised immune system. Without that posted notice, none of them had the information needed to make an informed choice about what to order.
The Longer Record
The July 2026 inspection was not an aberration. State records show Carolines Cafe has been inspected 26 times and has accumulated 192 total violations across that history. The cafe has never been emergency-closed.
The pattern of high-severity violations runs continuously through recent years. In July 2025, inspectors found seven high-severity violations and one intermediate, the single worst inspection count in the recent record. In January 2024, the cafe logged five high-severity and three intermediate violations. In August 2023, it was four high-severity and four intermediate.
The only inspection in the recent history that came back clean was in May 2022, when inspectors found zero high-severity violations and one intermediate. Every inspection since has included at least two high-severity citations.
That means the July 2026 visit, with its six high-severity violations, sits inside a four-year stretch in which the cafe has never gone a full inspection cycle without serious findings. The chemical storage violations and the handwashing failure are new additions to the record. The management failure citation is not: the person-in-charge violation has appeared in multiple prior inspections.
Still Open
Carolines Cafe sits on Duval Street, the main commercial corridor in Key West, drawing foot traffic from tourists and locals throughout the day. On July 21, 2026, state inspectors walked out of that kitchen having documented six high-severity violations, two intermediate violations, toxic chemicals stored near food, and no functioning handwashing infrastructure.
The cafe was not closed.
State rules allow inspectors to order an emergency closure when conditions pose an immediate threat to public health. The record shows that threshold was not reached, or not applied, on a day when inspectors found chemicals improperly stored alongside food in a kitchen where surfaces had not been sanitized and no one in charge was present to oversee operations.
The 192 violations across 26 inspections remain on file. The restaurant remained open on Duval Street.
