Facility has not registered the facility as required with The FDA. o INSPECTION: SEAFOOD HACCP Violation No. Citation Description and Observation COS 123.11(b) Citation Description: Sanitation conditions and practices not monitored with sufficient frequency to assure conformance with Current Good Manufacturing Practices Including: Cleanliness of food contact surfaces. 21 CFR 123.11(b) Observation: Specifically while conducting inspection on 07-11-23 observed, while in the seafood processing area, found the shovel used for icing down fishery products pitted and scored throughout the handle and shaft. o 123.11(b) Citation Description: Sanitation conditions and practices not monitored with sufficient frequency to assure conformance with Current Good Manufacturing Practices including: Exclusion of pests. 21 CFR 123.11(b) Observation: Specifically while conducting inspection on 07-11-23 observed, the door between the distribution bay doors maintained open throughout the duration of the inspection. o 123.11(b) Citation Description: Sanitation conditions and practices not monitored with sufficient frequency to assure conformance with Current Good Manufacturing Practices including: Protection of food, food packaging material, and food contact surfaces. 21 CFR 123.11(b) Observation: Specifically while conducting inspection on 07-11-23 observed, located inside the main walk in freezer observed heavy condensation build up along the ceiling fixtures, alarms, sprinklers, air blower, also observed condensation build up on pallet of Salted Pollock, no product found adulterated., cos the bagged ice and pallet of Salted pollock were moved, relocated. x Print Date: 7/12/2023 Page 1 of 3 123.11(b) Citation Description: Sanitation conditions and practices not monitored with sufficient frequency to assure conformance with Current Good Manufacturing Practices including: Maintenance of hand washing, hand sanitizing, and toilet facilities. 21 CFR 123.11(b) Observation: Specifically while conducting inspection on 07-11-23 observed, located in the ready to eat seafood processing room found a push cart being stored directly in front of the hand sink used for washing hands. o 123.6(b) Citation Description: *Procedures listed in HACCP plan not followed for monitoring. 21 CFR 123.6(b) Observation: Specifically procedures listed in your haccp plan of transportation records show the product was held below 40 degrees during the transport duration was not conducted for the pasteurized crab meat found in the storage, product was tempted below 40 degrees during visit. o 123.8(a) Citation Description: Process monitoring equipment is not calibrated to ensure that it reads accurately. 21 CFR 123.8(a)(2)(ii) Observation: Specifically the process monitoring equipment used for the continuous monitoring has not been calibrated for the year, to ensure equipment is properly operating. o 123.8(b) Citation Description: *Immediate corrective action not taken when verification procedure revealed the need to take a corrective action. 21 CFR 123.8(b) Observation: Specifically while reviewing continuous monitoring records for the dates July 8 and July 10 observed at the seafood control recorder showed that there was various spikes of temperature above 40 degrees and no corrective actions were taken or noted, product was checked during visit and no seafood product found above 40 degrees during the visit. o
