Specifically, observed 76 boxes containing 6 bags with 5lbs of shredded low moisture mozzarella cheese stored on the upper shelves on the north east side in the walk-in freezer. No invoices to verify proof of source was provided. Operator stated that cheese were not for the firm, but were being stored on site for another party. A Supplemental Report was also issued during the visit which includes important information for management.

Specifically; employee was not able to confirm that pest control is being performed regularly at the facility o 117.40 Citation Description: Food-contact surfaces not maintained to protect food from allergen cross-contact and from being contaminated by any source, including unlawful indirect food additives. 21 CFR 117.40(a)(6) Observation: Specifically; observed dirty and beyond cleanable towels being used to clean or to hold equipment on stainless steel table surface by personnel in the production room o 117.40 Citation Description: Equipment not installed so as to facilitate the cleaning and maintenance of the equipment and of adjacent spaces. 21 CFR 117.40(a)(3) Observation: Specifically; observed ware washing compartment sinks in the processing room and the production are not being used according as labeled Wash, Rinse, Sanitize and furthermore lack of sanitizing products have not been replenished, therefore equipment and utensils being used for food production is not properly washed. o 117.40 Citation Description: Food contact surfaces not corrosion-resistant. 21 CFR 117.40(a)(4) Observation: Specifically; observed rusted metal guard installed on the small floor mixer where cheese and dough are mixed.
