Comments: The establishment has submitted supporting documents regarding the employee rest rooms opening to the processing area in the back of the establishment. The documents have been submitted for plan review approval. The food permit has not been issued.

Comments: The produce production area is missing hand wash sink, a dedicated hand wash sink is required. The employee rest rooms located in the back ware house are located on the second floor. There is a open stair well that leads to the rest rooms, and the rest rooms open directly above the processing area. The rest rooms cannot open directly into the processing area. The manager has been advised that different documents for this situation are required. These documents usually include an air flow balance test result and a letter by the certified person explaining the results and how the new positive/negative air flow will work in the area to prevent contamination.
